Public Awareness Exhibit Request Form
Affiliate and Sexual & Domestic Violence Advocacy members of the Action Alliance may reserve Action Alliance public awareness exhibits to display in your community. To learn more about becoming a member, visit www.vsdvalliance.org.
The "Art of Surviving" and "Beating Hearts: Stories of Domestic Violence" exhibits require Action Alliance staff transport and/or set-up. For this reason, we ask you to contribute $100 to help defray costs for travel and materials.
By submitting this form, you are submitting a REQUEST only. An Action Alliance staff person will contact you to confirm whether or not the exhibit is available for the dates requested.
You will be asked to submit the $100 reservation fee online after confirming the exhibit dates.
Note: To borrow the SILENT WITNESS exhibit, contact the Action Alliance at 804.377.0335 or firstname.lastname@example.org. You do not need to complete this form to borrow the Silent Witness exhibit.

We ask that you reserve the Art of Surviving for at least a 1-month exhibit period; Beating Hearts for at least a 1-week exhibit period.
